Overview
Crawford County (the “County”) is issuing this Request for Proposals (the “RFP”) for design and remodel of the Crawford County Community Health New Location (City Center) at 115 N 14th Street, Denison, Iowa.
The features Crawford County is interested in but not limited to:
- First Floor
- East entrance – Gate at the top of the stairs.
- Office conversions – (7 rooms -105,106,110,114,115,116,120) remove sink/cabinets. Procedure room #120 leave the upper cabinets. (On drawing {#1}) Flooring replacement in offices where sink/cabinets are removed. Repair walls due to removal of sink/cabinets. Cap off plumbing.
- Install 2 doors (solid core w/frames) to close off corridor 107 & 134 (on drawing {#2})
- Restroom east wall add door (solid core w/frame) (on drawing {#3}) ADA compliant (ANSI)
- Create a private area. Add to existing counter (on drawing {#4})
- Wall covering is peeling and/or bubbling. Please repair or remove. The entire 1st floor will be painted.
- Remove vinyl letter on receptionist area. Remove logo wall hanging and repair.
- Room #127- remove shelving and bulletin board.
- Remove wall between room 118 & 119.
- Remove wall bumpers and monitor mounts from walls, random locations. Repair walls after removal.
- Exam room #102, remove white boards and cork boards
- Basement
- Repair drop ceilings-disposal /clean
- ADA accessible door – South building entrance
- Remove hanging dental/surgery lights.
- Restroom –ADA compliant (ANSI)
- Generator properly sized and location approved by County. To include transfer switch and wiring.
- Smoke & CO Detector (both floors)
- DMU will install a single meter for this building. Four breaker boxes will be reduced to 1 breaker box. The four current meters will be eliminated.
- Server room
- Steel door & steel jam
- block walls (south and east)
- 30-amp service (twist lock 120v (2)
- dropped ceiling
- Separate HVAC system
- Lights & standard duplex installed
- Redirect plumbing that runs over the server room.
- Building
- Repair retaining wall south end of building.
- Sidewalk repair and west parking
- Window repair- trim & paint. Possible rot
- Rain gutters repair
- South parking, ADA compliance.
Site Visits For prospective bidders only. A date for site walks will be scheduled for Nov 17, 2022, 10am. All vendors planning to submit a proposal must have a representative on hand for this activity. Please meet at 115 N 14th St, east entrance.
Sealed proposals will be received by the County until Dec 8, 2022, 3pm, in the Crawford County Auditor’s Office. No late submissions.

The County reserves the right to reject any and all proposals received pursuant to this RFP and to waive or not waive any informalities or irregularities in proposals or proposal procedures, and to accept or further negotiate cost terms or conditions of any proposal determined by the County to be in the best interest of the County even though not the lowest cost proposal.
Mandatory Requirements and Conditions
A response to this RFP must be sealed and must include and/or comply with the following:
- A cover letter designating the name, address and telephone number of the person or persons available for contact concerning the proposal.
- Descriptions and/or statements regarding labor, installation, warranties, shipping/delivery, and experience regarding prior design and/or installations of similar systems.
- Statements regarding manufacturer authorization regarding proposed products.
- A timeline regarding provision of services. Work must be completed by March 1, 2023.
- Proof of Insurance
- References.
Opening of Bids and Selection of Proposal
Proposals will be opened by the County December 9, 2022.
Evaluation Criteria
In determining the most favorable proposal the County shall consider the following:
- Terms of services to be provide and materials to be used.
- Pricing
- References
- Prior knowledge or prior dealings of the proposer, as the knowledge or dealings relate to official business of the County.
- Any other information and /or factors that the County considers relevant.
Contract will be announced December 13, 2022, at the Board of Supervisors meeting.
